NGCC Non-Prescription Form – Please use this form for any Non-Prescription medications
Individual Health Care Plan Form – Complete this form for children with diagnosed allergy, condition or restrictions.
General Registration Packet - Please note, you will need to sign a Parent-Guardian Emergency Evacuation Form that is school-specific in addition to this packet
Drop-In Care Registration Packet - Please note, any drop-in care must be pre-arranged with school prior to attendance. This packet is only the basic information needed to arrange care. More forms will need to be completed prior to attendance.
Outside Transportation Form – Please complete this form if you are working with your public school or a private contractor for transportation of your child to and/or from school.
